Advocacy & Wellness Support Session

Service Description

This service provides personalized support for seniors and their families, including follow-up calls after visits with your loved one. During these sessions, we discuss ongoing advocacy needs, answer questions, address concerns, and review any aspects of continued care management. Detailed notes from attended appointments, along with summaries of our discussions and recommendations, will be provided following each phone call or in-person meeting to ensure you remain informed and supported throughout the care process.
